The Abbotsford Division of Family Practice (ADoFP) is a non-profit organization supporting physicians to improve health access and equity for Abbotsford residents. Working with the Family Practice Services Committee, Fraser Health, community groups, universities, the City of Abbotsford, and the provincial government, we bring forward programs and services that benefit and promote health and wellness in our community.

Primary Care Network
The Abbotsford PCN is a multi-agency collaborative program with the goal of strengthening primary health care in the community by supporting patients, providers, and practices through team-based care. Read More
Long-term Care Initiative
The Long-term Care Initiative (LTCI) is Abbotsford Division’s largest program, implementing local solution to deliver services for long-term care patients. The program has 11 long-term sites across Abbotsford and about 32 members participating. Read More
Resident Engagement

Recognizing the need to engage doctors at the beginning of their medical career journey, Abbotsford Division has established a dynamic resident engagement program to support the success of medical residents, who will be the future physicians in our community.
Gender-affirming Care
The Transgender Patient Pathway project was designed to address gaps and promote awareness of services and referral pathways for transgender care in Fraser East. The project supported education events and supported the development of a community of practice. Read More
